pay-for-perfomance movement in health care is misguided, Joseph Leutzinger, president of the Academy for Health & Productivity Management, said.

Questioning why people should receive extra rewards for doing their job well, Leutzinger compared the idea to his own occupation, dryly noting, "As a single business owner, if I don't perform, I don't get paid."

"I just think [pay-for-performance] is the wrong paradigm," he added. "Physicians are already in the top percentile of incomes. Where else can you go?"

What Leutzinger does support, though, is "health and productivity management," which combines such things as encouraging healthy lifestyles, preventing injuries, intervening early to address health needs and managing chronic illnesses to keep employees as healthy as possible, minimize health care costs and maximize productivity.
